Qurrah LLC (“Qurrah,” “we,” “us,” or “our”) respects the privacy of every guest, prospective  guest, parent or guardian, and website visitor who entrusts us with personal information. 

This Privacy Policy explains how Qurrah collects, uses, protects, retains, and discloses personal  information through our website, communications, scheduling system, intake and consent forms,  therapeutic records, and payment systems. 

This policy applies to information collected through qurrahwellness.com, through Qurrah’s  Massagebook Scheduling system, and through other interactions with Qurrah. 

1. Information We Collect 

The information Qurrah collects depends on how you interact with us. 

Website Contact Form 

When you contact Qurrah through our website, we may collect: 

• Your name 

• Your email address 

• Your telephone number 

• Your preferred method of contact 

• Your preferred contact time 

• Your reason for reaching out 

• Whether you are a new or returning guest 

• Your acknowledgment that Qurrah serves women exclusively 

• Your acknowledgment that the contact form is not intended for urgent or emergency  medical or health concerns 

Our general website contact form is intended for ordinary inquiries and communication. Please  do not use it to submit detailed medical histories, highly sensitive health information, payment card information, or emergency concerns. 

Health and intake information requested by Qurrah should be submitted through the designated  Massagebook forms provided for that purpose. 

Scheduling and Guest Information 

Qurrah uses Massagebook Scheduling to manage appointments and guest information. When you schedule a sanctuary visit, Qurrah or Massagebook may collect information such as:

• Your name 

• Your email address 

• Your telephone number 

• Your date of birth 

• Your address 

• Appointment selections and preferences 

• Appointment history 

• Communication preferences 

• Information related to sanctuary paths, packages, gift certificates, or other service  arrangements 

• Payment and transaction information 

• Information you provide through required scheduling or intake forms Massagebook may also process information necessary to provide appointment confirmations, reminders,  scheduling functions, and other services associated with your appointment. 

Intake, Consent, and Therapeutic-Care Information 

To provide safe, individualized therapeutic care, Qurrah may collect information such as: 

• Date of birth 

• Home address 

• Emergency-contact information 

• Physician or healthcare-provider information 

• Health history 

• Medical diagnoses and health conditions 

• Medications 

• Allergies and sensitivities 

• Pregnancy status 

• Injuries 

• Surgeries 

• Areas of pain, tension, discomfort, or concern 

• Mental or emotional health information relevant to care 

• Treatment goals and preferences 

• Contraindications or precautions 

• Consent choices 

• Information regarding specific healing modalities 

• Electronic acknowledgments or signatures 

• Session notes 

• Treatment records 

• Professional observations 

• Changes in health status 

• Information you voluntarily share before, during, or after a sanctuary visit when relevant  to your care 

Qurrah seeks to collect only information reasonably necessary to provide safe and appropriate  care, document informed consent, maintain professional records, and fulfill legitimate legal and  business responsibilities.

Information Concerning Minor Guests 

Qurrah may provide services to guests under 18. 

When a minor receives services, Qurrah may collect information about both the minor guest and  her parent or legal guardian. 

A parent or legal guardian must: 

• Complete or approve required intake, consent, and service forms 

• Provide accurate health and contact information 

• Give permission for the minor to receive services 

• Remain present during the minor’s sanctuary visit 

Payment Information 

Qurrah uses third-party payment-processing services to process card and electronic payments. Depending upon the method of payment, a payment provider may collect: 

• Cardholder name 

• Billing information 

• Card or account information 

• Transaction amount 

• Payment status 

• Refund information 

• Information used to detect or prevent fraud 

The payment processor used for a particular transaction will be identified during the payment  process. 

Qurrah does not intentionally record or store complete payment-card numbers within its  therapeutic client records. Payment processors handle payment credentials according to their  own privacy, security, and payment-processing practices. 

Qurrah may retain information such as transaction dates, amounts, receipts, payment status,  refunds, and other accounting records. 

Marketing Information 

If you choose to subscribe to communications from Qurrah or otherwise expressly opt in, we  may collect: 

• Your name 

• Your email address 

• Your telephone number, when applicable 

• Your communication preferences 

• Information documenting your consent

• Records of an unsubscribe or opt-out request 

Marketing and informational communications may include: 

• Newsletters 

• Educational materials

Consent to receive marketing communications is voluntary and is not required to receive  therapeutic services. 

Testimonials, Reviews, Photographs, Audio, and Video With your separate written consent, Qurrah may collect, use, or publish: 

• Written testimonials 

• Statements about your experience 

• Your first name, initials, or another approved identifier 

• Before-and-after information 

• Photographs 

• Audio recordings 

• Video recordings 

Providing this consent is voluntary and is not a condition of receiving care. Information Collected Automatically Through the Website 

When you visit Qurrah’s website, Squarespace and other website-service providers may  automatically collect certain technical information, which may include: 

• Internet Protocol address 

• Browser type 

• Device type 

• Operating system 

• General geographic region 

• Referring website 

• Pages viewed 

• Links selected 

• Dates and times of website visits 

• Website-performance information 

• Cookie or similar technology identifiers 

This information may be collected even when you do not complete a form or schedule a  sanctuary visit. 

2. How We Use Personal Information

Qurrah may use personal information to: 

• Respond to inquiries 

• Communicate with prospective and returning guests 

• Schedule, confirm, change, or cancel appointments 

• Send appointment confirmations and reminders 

• Provide required intake and consent forms 

• Create and maintain guest records 

• Review health information 

• Identify possible precautions or contraindications 

• Plan and provide individualized therapeutic care 

• Document consent 

• Maintain session and treatment records 

• Maintain accurate professional and business records 

• Process payments, refunds, and transactions 

• Respond to concerns, questions, or requests 

• Communicate with a parent or legal guardian when appropriate 

• Send newsletters, educational information, announcements, or promotional  communications when consent has been given 

• Publish testimonials or media when separate written consent has been provided • Maintain the security and proper operation of Qurrah’s website and systems • Prevent fraud, unauthorized access, misuse, or other harmful activity • Protect the health, safety, rights, and property of Qurrah, its guests, and others • Comply with professional, legal, regulatory, tax, accounting, and recordkeeping  obligations 

• Establish, exercise, or defend legal rights or claims 

Qurrah does not use health histories, intake responses, treatment records, or session notes for  unrelated advertising or marketing purposes. 

3. Health Information and the Website 

Qurrah maintains an important distinction between its general website and the systems used to  collect therapeutic-care information. 

The Squarespace contact form is intended for general communication and should not be used to  submit detailed medical or health information. 

Health histories, consent information, contraindication screening, and other information  requested for therapeutic care are collected through Qurrah’s designated Massagebook intake forms. 

Guests should use the forms and communication methods provided by Qurrah for the type of  information being requested. 

4. Privacy of Health and Treatment Information

Qurrah considers health histories, intake forms, consent records, session notes, treatment  information, and related therapeutic-care information confidential. 

Qurrah does not sell this information or use it for unrelated marketing purposes. Qurrah will disclose health or treatment information only when: 

• You have authorized or directed the disclosure; 

• Disclosure is reasonably necessary to provide care that you have requested; • Disclosure is reasonably necessary to obtain immediate medical assistance or protect  health or safety; 

• Disclosure is required or permitted by applicable law; or 

• Disclosure is otherwise necessary to fulfill a professional or legal obligation. Guests are encouraged to provide accurate and current health information. Incomplete or  inaccurate information may affect Qurrah’s ability to determine whether a particular service or  healing modality can be provided safely. 

5. Health Insurance 

Qurrah does not accept or bill health insurance. 

Qurrah does not routinely transmit guest health information to health-insurance companies for  claims processing. 

If a guest requests that information be provided to another healthcare provider or another  authorized person, Qurrah may do so with appropriate authorization and subject to applicable  professional and legal requirements. 

6. How We Share Information 

Qurrah does not sell or rent personal information

Qurrah does not provide guest information to other businesses for their independent marketing  purposes. 

Limited information may, however, be processed by third-party service providers that Qurrah  relies upon to operate the sanctuary and provide requested services. 

Service Providers 

Current or anticipated service providers may include: 

Squarespace, which provides Qurrah’s website platform and hosting • Massagebook Scheduling, which provides online scheduling, intake forms, guest-management  functions, appointment communications, and related services 

GoDaddy, which provides domain-registration and domain-management services

Google Workspace and Gmail, which provide Qurrah’s business email services • AT&T, which provides telephone, text-message, and voicemail services • Third-party payment processors used to process electronic or card payments • Professional accounting, legal, technology, security, or other service providers when  reasonably necessary to operate Qurrah 

These providers may process only the information involved in providing the applicable service.  Their handling of information is also governed by their own privacy policies, agreements,  security practices, and legal obligations. 

Future Qurrah Therapists 

At present, Qurrah’s owner is the only person within Qurrah who has routine access to guest  therapeutic records. 

If Qurrah engages additional therapists in the future, information relevant to a guest’s care may  be made available to a therapist when: 

• The therapist is involved in providing care to that guest; 

• The information is reasonably necessary for that care; and 

• Appropriate guest, parent, or guardian consent has been obtained, unless disclosure is  otherwise permitted or required by law. 

Access will be limited to information reasonably necessary for the therapist’s professional  responsibilities. 

At Your Direction 

Qurrah may disclose information when you specifically ask or authorize us to do so. This may include providing information to: 

• Another massage therapist 

• A physician 

• Another healthcare professional 

• A parent or guardian 

• Another person designated by you 

Qurrah may request written authorization before releasing therapeutic records. Legal, Health, or Safety Requirements 

Qurrah may preserve or disclose information when reasonably necessary to: 

• Comply with applicable law 

• Comply with a court order, subpoena, or other lawful legal process 

• Respond to an authorized government or regulatory request 

• Fulfill professional licensing obligations 

• Obtain emergency medical assistance

• Protect the health or safety of a guest or another person 

• Investigate suspected fraud, misconduct, or unlawful activity 

• Protect Qurrah’s legal rights, property, safety, or operations 

• Establish, exercise, or defend a legal claim 

Qurrah will seek to limit any disclosure to information reasonably necessary under the  circumstances. 

7. Massagebook Scheduling 

Qurrah uses Massagebook Scheduling as its primary scheduling and guest-management platform. Massagebook may be used to manage: 

• Online scheduling 

• Guest profiles 

• Intake forms 

• Health-history questionnaires 

• Safety screening 

• Consent forms and acknowledgments 

• Appointment histories 

• Session-related information 

• Appointment confirmations 

• Appointment reminders 

• Packages or recurring care arrangements 

• Payment-related functions 

• Other administrative functions related to a sanctuary visit 

Information submitted through Massagebook is also subject to Massagebook and Squarespace’s applicable  privacy, security, and service terms. 

Qurrah may modify the way Massagebook is configured as its services develop. If Qurrah later changes  to another platform in a way that materially affects how guest information is collected, stored,  used, or disclosed, this Privacy Policy will be updated accordingly. 

8. Email, Telephone, Text, and Voicemail 

Qurrah’s business email is provided through Google Workspace and Gmail. Qurrah may communicate with guests by email, telephone, text message, or voicemail regarding: 

• Responses to inquiries 

• Scheduling 

• Appointment confirmations 

• Appointment reminders 

• Forms requiring completion 

• Changes to appointments

• Changes to services or policies 

• Payment or account matters 

• Follow-up information 

• Other communications reasonably related to requested services 

Email, text messaging, and voicemail are convenient forms of communication, but no ordinary  electronic communication method can be guaranteed to be completely secure. 

Guests should avoid sending detailed medical histories, complete payment-card numbers,  government identification numbers, or other highly sensitive information through ordinary email,  text message, or voicemail unless Qurrah specifically requests the information and provides an  appropriate method for submitting it. 

Health-related information requested as part of intake should ordinarily be submitted through the  designated Massagebook forms. 

9. Marketing Communications 

Qurrah may send newsletters, educational materials, special announcements, and promotional  communications to individuals who have chosen to receive them. 

You may withdraw marketing consent at any time by: 

• Using an unsubscribe method provided in the communication, when available; • Replying to the communication with a request to unsubscribe; or 

• Contacting Qurrah at hello@qurrahwellness.com.

Withdrawing marketing consent does not prevent Qurrah from sending non-promotional  communications reasonably necessary to: 

• Respond to your inquiry 

• Manage an appointment 

• Send appointment reminders 

• Provide requested services 

• Communicate health or safety information 

• Address a payment or account matter 

• Provide a legally required notice 

• Notify you of a material change to Qurrah’s policies 

Qurrah may retain the minimum information reasonably necessary to honor an opt-out request  and prevent an individual from being unintentionally returned to a marketing list. 

10. Cookies and Website Analytics 

Qurrah’s Squarespace website may use cookies and similar technologies. 

These technologies may be used to: 

• Operate the website

• Maintain website security 

• Remember website preferences 

• Improve website performance 

• Understand general visitor activity 

• Measure website traffic 

• Identify technical problems 

Some technologies are necessary for the website to operate. Others may be used for analytics or  performance purposes. 

Where Qurrah provides cookie-management choices, visitors may use those controls to manage  available preferences. Visitors may also be able to control cookies through their browser settings. 

Disabling certain cookies may affect how some website features operate. 

Qurrah does not sell website-browsing information. 

Qurrah does not currently use guest health or treatment information for targeted or personalized  advertising. 

If Qurrah later introduces additional analytics, advertising pixels, targeted advertising, or similar  technologies that materially change its privacy practices, this policy will be updated as  appropriate. 

11. Children’s and Minors’ Privacy 

Qurrah’s website is intended for adults and is not directed to children under 13. 

Children under 13 should not independently submit personal information through Qurrah’s  website, contact form, email, or scheduling system. 

When services are requested for a minor, a parent or legal guardian should provide the necessary  information on the minor’s behalf. 

Qurrah may provide services to guests under 18 only when a parent or legal guardian: 

• Completes or approves required forms; 

• Provides consent for services; and 

• Remains present during the minor guest’s sanctuary visit. 

A parent or legal guardian may contact Qurrah regarding information maintained about a minor  guest and may request appropriate access, correction, or other action subject to applicable  professional and legal recordkeeping requirements. 

If Qurrah becomes aware that personal information has been submitted directly by a child under  13 without appropriate parental involvement, Qurrah will take reasonable steps to address the  information appropriately.

12. Testimonials, Reviews, Photographs, Audio, and Video 

Qurrah will obtain separate written consent before publishing material that identifies or may  reasonably identify a guest. 

A guest may choose what information, if any, Qurrah is permitted to use. 

Refusing permission for publication will not affect eligibility for services or the care a guest  receives. 

A guest may later withdraw consent for future use by contacting Qurrah. 

After receiving such a request, Qurrah will make reasonable efforts to discontinue future  publication or use under Qurrah’s control. 

Withdrawal of consent may not enable Qurrah to retrieve: 

• Printed materials already distributed 

• Content already viewed, downloaded, copied, or shared by others 

• Information independently posted by a guest on a public review platform • Archived records Qurrah is required or reasonably permitted to retain Qurrah will not knowingly alter a testimonial in a way that misrepresents the meaning of the  guest’s statement or experience. 

13. How Long We Retain Information 

Qurrah retains personal information only for as long as reasonably necessary to provide services,  maintain appropriate professional and business records, fulfill the purposes described in this  policy, comply with applicable requirements, resolve disputes, and protect the rights and safety  of Qurrah and its guests. 

Qurrah generally follows these retention practices: 

• Guest intake, consent, health-history, appointment, treatment, and session records: Generally retained for seven years after the guest’s last service, and not for less than  any period required by applicable law. 

• Records concerning minor guests: May be retained longer when appropriate because of  the guest’s age or applicable professional or legal requirements. 

• Financial, payment, transaction, and accounting records: Generally retained for  approximately seven years or longer when required. 

• General contact-form inquiries from individuals who do not become guests: Generally retained for up to 12 months after the last meaningful communication. • Routine scheduling and administrative communications: Generally retained only as  long as reasonably necessary unless they become part of a guest-care, payment,  complaint, safety, or legal record.

• Health- or treatment-related communications: May become part of the guest’s  professional record and be retained according to the applicable guest-record retention  period. 

• Marketing-consent records: May be retained while an individual remains subscribed  and for a reasonable period afterward. 

• Marketing opt-out records: The minimum information reasonably necessary may be  retained to ensure the opt-out request continues to be honored. 

• Testimonial and media consent records: May be retained while the material is being  used and for a reasonable period afterward. 

Information may be retained longer when reasonably necessary to: 

• Comply with law or professional requirements 

• Respond to a complaint, investigation, or legal claim 

• Resolve a payment dispute 

• Prevent fraud or misuse 

• Preserve the integrity of professional records 

• Protect health or safety 

• Establish, exercise, or defend legal rights 

When information is no longer reasonably necessary, Qurrah will take reasonable steps to delete,  anonymize, or otherwise appropriately dispose of it. 

14. Information Security 

Qurrah uses reasonable administrative and technical safeguards appropriate to the nature of the  information it maintains. 

Current safeguards include: 

• Two-factor authentication 

• Device passcodes 

• Password-protected accounts 

• Restricted access to guest records 

• Use of established service providers 

• Limiting access to information according to legitimate care and business needs At present, Qurrah’s owner is the only person within Qurrah with routine access to therapeutic  guest records. 

No website, scheduling platform, email service, telephone system, payment system, device, or  method of electronic transmission can be guaranteed to be completely secure. Qurrah therefore  cannot promise absolute security. 

If Qurrah becomes aware of a security incident involving personal information, Qurrah will  investigate the circumstances and provide notification when required by applicable law. 

15. Your Privacy Choices and Requests

Subject to applicable law and professional recordkeeping requirements, you may contact Qurrah  to: 

• Request a copy of information Qurrah maintains about you 

• Ask that inaccurate or incomplete information be corrected 

• Request eligible deletion of information 

• Withdraw marketing consent 

• Withdraw permission for future use of a testimonial, photograph, audio recording, or  video 

• Ask questions about how your information is collected, used, stored, retained, or  disclosed 

Qurrah may request information reasonably necessary to verify your identity before releasing,  changing, or deleting information. 

A parent or legal guardian may make an appropriate request on behalf of a minor guest. Qurrah  may verify the requester’s identity and authority before responding. 

Corrections to Professional Records 

When a requested correction concerns a professional treatment, health, intake, consent, or  session record, Qurrah may preserve the original entry and add a dated correction, clarification,  or amendment rather than erasing or rewriting the original record. 

Deletion Requests 

Qurrah may be unable to delete certain information when continued retention is reasonably  necessary to: 

• Meet professional recordkeeping requirements 

• Comply with tax, accounting, or other legal responsibilities 

• Preserve an accurate treatment or consent record 

• Resolve a dispute 

• Prevent fraud 

• Protect health or safety 

• Establish, exercise, or defend a legal claim 

When complete deletion is not appropriate, Qurrah may restrict use of the information or retain  only what remains reasonably necessary. 

16. Third-Party Websites and Services 

Qurrah’s website or communications may contain links to external websites, Massagebook, payment  processors, social-media platforms, review sites, maps, or other third-party services. 

Qurrah does not control the privacy, security, content, or data-handling practices of independent  third parties.

Information submitted directly to another provider may be subject to that provider’s own privacy  policy, terms, and security practices. 

Guests are encouraged to review the privacy information provided by third-party services before  submitting sensitive personal or payment information. 

17. Changes to This Privacy Policy 

Qurrah may update this Privacy Policy as its services, business practices, technology providers,  or legal obligations change. 

The effective date displayed at the beginning of this policy identifies when the current version  takes effect. 

If Qurrah makes a material change affecting how personal information is collected, used,  retained, or disclosed, Qurrah may provide notice through one or more appropriate methods,  including: 

• Posting notice on the Qurrah website 

• Sending an email 

• Providing notice through Massagebook 

• Presenting an updated acknowledgment or consent 

• Communicating directly with affected guests 

Additional consent will be obtained when required.
